Hazardous Waste Pick up Request Form
小蓝视频 contracts with a professional waste company for all hazardous waste disposal. The Chemical Hygiene Officer is responsible for removal of hazardous waste from the SAA's. Fill out the form below to request pick up for full or no longer needed containers.
Containers will only be picked up if the Hazardous Waste label is complete and ledgibe and a "Ready for Pick-up" sticker is afixed to the the bottle. Stickers are available through the Office of Environmental Health and Safety (EHS) for Academic Affairs.
